I do hereby acknowledge that of my own free will and desire, I have contracted with the league for the instruction, training and playing of my child(ren) in football and that I have read and understood the ‘Waiver and Indemnity’, ‘Player Code of Conduct’ and ‘Parents, Caregivers and Supporters Code of Conduct’ on the back of this form.

WAIVER AND INDEMNITY

“Football”               Refers to                - Australian Rules Football (AFL)
“League”               Refers to                - West Kimberley Football League
“Child”                    Refers to                - Child or ward (Whichever is applicable)

I am aware that playing football, observing football, learning to play football and training to play football, and participating in any activity carried out by the League, are activities that inherently involve risk, and that in understanding these activities my child(ren) does so at his/her own risk.

I am also aware that, as a condition of my child(ren) admission to membership of the League, its office bearers, officials, coaches, managers, umpires, members and/or agents are absolved from all liability arising from injury or damage howsoever caused. That injury or damage may arise out of membership of the league, playing football, observing football, learning to play football and training to play football, or negligence of any nature whatsoever on the part of the league, their representatives, office bearers, officials, coaches, managers, umpires, members and/or agents.

I understand and agree that nothing in this agreement purports to exclude any liability that may be owed by the League and their representatives pursuant to the provisions of any relevant Statutory Act’s covering these activities.  To the extent that any part of this agreement may contravene any Statutory Act’s I agree that part of the agreement may be severed and rendered void but the rest of the agreement will remain valid and continue to have effect.

Players Code of Conduct

·         Play by the rules and within the ‘spirit of the game’.
·         Play for the ‘fun of it’ and not just to please parents, coaches and teachers.
·         Shake hands with your opponent after the game.
·         Control your temper. Verbal abuse of officials or other players, deliberately fouling or provoking an opponent and throwing equipment is neither acceptable nor permitted in any sport.
·         Work equally hard for yourself and your team. Your team’s performance will benefit and so will you.
·         Be a good sport. Admire all good passages of play whether they are by your team or the other team.
·         Treat all players as you would like to be treated. Do not interfere with, bully or take unfair advantage of another player.
·         Co-operate with the umpires, your coach, team mates and opponents. Without them there would be no game.

Parents, Caregivers and Spectators Code of Conduct

·         Remember children play sport for their enjoyment not yours.
·         Focus upon the child’s efforts and performance rather than the overall outcome of the event. This assists the child in setting realistic goals related to their ability by reducing the emphasis on winning.
·         Encourage children to always participate according to the rules.
·         Never ridicule or yell at a child for making a mistake or losing a game.
·         Remember children learn best by example.
·         Applaud good play by all teams.
·         Encourage children to participate, do not force them.
·         Support all efforts to remove verbal and physical abuse from sporting activities.
·         Respect official’s decisions and teach children to do likewise.
·         Show appreciation of volunteer coaches, officials and administrators. Without them your child could not participate.
·         If you disagree with an official, raise the issue through the appropriate channels rather than question the official’s judgment and honesty in public. Remember that most officials give their time and effort for your child’s involvement.
·         Consumption of alcohol is inappropriate at all junior games.

END.
